To schedule your appointment please fill out this form. All our appointments are currently carried out online.

You need this information to complete the form: your driver's license, your insurance card, and an email address that you check frequently throughout the day.

Once you have filled the form we will send you a confirmation email with a username and password to access the online platform. Along with the email confirming your appointment, you will receive also 3 short forms to be completed online (no printing required). It should only take few minutes to complete these forms, however, please take as long as you need to properly complete them and let your doctor know about your current situation. If you are a new patient your doctor has never met you: the more your write in the form the better.

Your appointment is scheduled. Shortly, you will receive a confirmation email and three online forms: complete all forms at least 24h before your appointment. If we do not receive these forms your appointment will be automatically canceled, with no exceptions.
